Dr. Johanna Mock-Ohnesorge from Heidelberg University Hospital said: “Although general anesthesia with intravenous and volatile anesthetics is well and safely established for extensive abdominal surgery, little is known about the effects of anesthesia on perioperative metabolism.

While in the past anesthetic procedures were the sole requirement for an operation, modern anesthesia has evolved into an individualized treatment to meet the individual needs of the patient in terms of metabolic, physiological and immunological integrity with a view to achieving a long-term clinical outcome.

Since immune cells, as natural killer cells or CD8 T cells, have the ability to identify and eliminate these malignant cells, the perioperative immune status plays an essential role in anti-tumor activities.

A variety of factors contribute to a complex modulation of immune function during surgery, e.g. the release of damage-associated molecular patterns from injured tissue and the release of metabolites of the organism in response to anesthesia and surgical stress.

The aim of this prospective, exploratory study is to gain information on longitudinal perioperative changes in the plasma metabolome during general anesthesia in patients with diagnosed pancreatic cancer, which will lead to a better understanding of personalized perioperative management.

The Mock-Unesorge research team concluded in their oncotarget research output that metabolomics represents a phenotype of all genetic, transcriptomic and post-translational modifications.

The perioperative metabolomic analysis enables a more detailed insight into the perioperative processes.

This meets individual needs and enables personalized anesthesia management.

In the field of surgical oncology, therapeutic windows with the best prerequisites for perioperative immunotherapy or chemotherapy could possibly even be identified.

Further controlled studies are urgently needed to compare the effects of different anesthetic procedures on the plasma metabolome in order to identify positive or harmful influences on the long-term clinical outcome.
